Acceptance of Terms
By accessing our website, scheduling an appointment, or receiving care from Whole Women’s Care MD, LLC, you agree to be bound by these Terms and Conditions.
About Our Practice
Whole Women’s Care MD, LLC is a direct-pay gynecology practice owned and operated by Dr. Maria Casiano, MD, FACOG, MSCP, DABOM, located at 3901 Mary Eliza Trace NW, Suite 202, Marietta, GA 30064. Dr. Casiano is licensed to practice medicine in the State of Georgia.
Direct-Pay Model — No Insurance Billing
Whole Women’s Care does not bill insurance companies directly. By scheduling an appointment, you acknowledge that:
Payment for all services is due in full at the time of service
We accept major credit/debit cards and HSA/FSA cards
We do not submit claims to insurance companies on your behalf
Upon request, we will provide a superbill for potential reimbursement — no guarantee of reimbursement is made
Laboratory testing, imaging, and medications are billed separately
Cancellation & No-Show Policy
Payment is collected at the time of booking. Cancellations made with prior notice will receive a full refund. Missed appointments without any prior notice (no-shows) will result in a $50 fee retained from the prepaid amount; the remaining balance will be refunded. Repeated no-shows may result in discharge from the practice at Dr. Casiano's discretion.
Scope of Services & Telehealth
Services are provided by Dr. Maria Casiano, MD, FACOG, MSCP, DABOM. Nothing on our website constitutes medical advice or establishes a physician-patient relationship. A physician-patient relationship is established only upon your first in-person visit. Telehealth services are available to established patients only — an in-person visit is required to establish care prior to telehealth services. Telehealth is subject to Georgia law including O.C.G.A. § 33-24-56.4.
Care Pathways and Package Purchases
Packages must be used within 12 months of purchase unless otherwise specified
Packages are non-refundable once any included service has been rendered
Unused portions may be refunded on a prorated basis at the practice’s discretion
Package pricing is subject to change — purchased packages honored at purchase price
Patient Responsibilities
To support the best possible care experience, patients are responsible for the following:
Providing accurate and complete information regarding health history, current medications, supplements, and any relevant prior medical records
Notifying the practice of cancellations or rescheduling requests at least 48 hours in advance, in accordance with the Cancellation & No-Show Policy above
Payment of all fees at the time of service, including any applicable procedure fees
Keeping contact information — including phone number, email address, and preferred pharmacy — current with the practice
Following through on referrals, laboratory testing, imaging, or other recommendations made during your visit, and understanding that outcomes cannot be guaranteed when recommended care is not pursued
Understanding that Whole Women's Care is not an emergency medical service — in the event of a medical emergency, patients should call 911 or go to the nearest emergency room
Communicating openly and honestly with Dr. Casiano so that care can be individualized to your full health picture

To the fullest extent permitted by Georgia law, Whole Women’s Care MD, LLC shall not be liable for indirect, incidental, or consequential damages. Nothing in these Terms limits liability for medical negligence or any liability that cannot be limited under applicable law.
Governing Law
These Terms are governed by the laws of the State of Georgia. Any disputes shall be resolved in the courts of Cobb County, Georgia.
